What is the EHR Care Stream app?

The EHR Care Stream app is a mobile extension of Yardi EHR, designed for nurses and senior living care staff. They can access resident health records, perform order resolutions and complete point-of-care charting directly from a smartphone or tablet.

How does the point-of-care charting work?

Caregivers can document care at the time and place services are delivered to residents. The app supports real-time recording of data and offline documentation. It automatically syncs when internet connectivity is restored.

What activities can care staff do in the EHR Care Stream app?

Gain visibility with resident summaries: The Resident Summary displays resident cards with counts for missed tasks, due tasks, PRN tasks and chart by exception tasks. Care staff can quickly assess and prioritize daily responsibilities.

View and create daily logs and progress notes: With appropriate permissions, care staff can view historical notes and create new daily logs and progress notes directly within the app.

Communicate through the message center: In the integrated Message Center, staff can view and send messages directly within the app.

Access information in less time and with improved navigation: Recent updates include performance improvements that deliver faster load times, smoother navigation and an enhanced mobile experience.
