Connected systems are essential for electronic health records (EHRs) in senior living. When clinical and operational systems share data effectively, senior living communities can deliver higher-quality care and reduce manual entry burdens. This visibility into resident health trends also supports value-based care programs.
Each quarter, we showcase how new Yardi EHR interface partners help senior living providers deliver more efficient, coordinated and person-centered care.
New summer 2026 EHR partners
Intrex
Intrex provides the Rythmos® safety system with continuous resident safety and operational clarity across apartments, senior living communities and beyond. Rythmos connects wearable, in-apartment and environmental signals into a unified system that delivers real-time awareness without limiting independence.
Intrex connects to Yardi EHR through the FHIR-based API to populate Yardi EHR community census data in their system based on routine daily actions. Intrex Rythmos receives resident census, room locations and clinical data, updated in near-real time when qualifying changes are made in Yardi EHR. Yardi EHR receives acknowledged fall alerts made by Rythmos, which generate incidents in Yardi EHR.
With shared information between Yardi EHR and Intrex Rythmos, communities can improve clinical efficiency and provide better resident care and safety. With the automated clinical data updates, staff have more time to engage with residents and provide care.
PalCare
PalCare, an end-to-end senior care technology solution for nurse calls in the senior housing industry, collects real-time data from nurse calls and analyzes trends. It gives insights into how to adjust resident care proactively, meet compliance standards and manage staff and resources.
Automated census management between Yardi EHR and the PalCare application syncs resident clinical data securely, including fall alerts sent to Yardi EHR from PalCare devices (Yardi EHR version 7.25).
Resident data updates flow seamlessly between systems to help reduce manual processes, improve accuracy and support smoother daily operations. This operational efficiency means communities experience alignment across teams, more confidence in resident data and workflows that support how communities actually operate.
TapestryHealth
TapestryHealth integrates data analytics, AI tools and remote monitoring to assist clinicians in improving resident care in assisted living communities. Data-driven solutions help clinicians take proactive measures to reduce hospital readmissions and improve care.
TapestryHealth uses the FHIR API to pull data from Yardi EHR into the Tapestry application for real-time updates to resident records entered by nursing staff. Clinician notes entered by a provider in TapestryHealth update a resident’s profile in Yardi EHR as progress notes or digital documents.
Communities using Tapestry and Yardi EHR together experience automated census management and a more cohesive care system between provider and community records.
Spring 2026 EHR partners
Amba
Amba collects real-time health and well-being data from discreet devices placed around residents’ rooms and sends it to a dashboard for caregivers. This includes mobility and wandering, fall detection, continence management and sleep activity. With this data, caregivers can deliver more responsive and personalized care to the residents.
To improve care and connect data between Yardi EHR and Amba’s patient monitoring tool, Amba connects to EHR through the FHIR-based API.
Resident census, room location and relevant clinical data can be synchronized with Amba. This reduces manual entry and helps caregivers act on current information. Without the need to jump from one system to another, staff have more time to engage with residents and provide care.
Centered Care
To prevent crises and hospitalizations, Centered Care is a proactive care model that closes gaps in follow-through with a shared view of resident changes between senior living communities and health care providers.
Centered Care uses the FHIR API to pull data from Yardi EHR into their application. In Yardi EHR, when care staff enter clinical patient data — such as medications, allergies and diagnoses — it updates the Centered Care system.
Yardi EHR clients can create a more cohesive care system, eliminating duplicative data entry.
In-House Health
In-House Health provides predictive, automated staff scheduling. The system uses census data to look at how many residents are in a community and then recommends how many staff members are needed and where.
In-House Health connects to Yardi EHR through the FHIR API and seamlessly creates a map of each community they pull data for. Yardi EHR clients can collect data more efficiently for the staff scheduling. And it’s easier to onboard new communities into the In-House Health landscape.
LifeLoop
LifeLoop is a proactive engagement and wellness platform that includes communication and personalization to help providers improve resident well-being and business outcomes.
Using the FHIR API, LifeLoop pulls in data from routine daily census actions in Yardi EHR to populate its community census.
Yardi EHR clients only need to manage resident census changes in one location, giving them more time to work directly with residents.
Sentrics
To help senior living operators run safer, smarter communities, Sentrics delivers a care intelligence platform combining emergency response, analytics and integrations with managed phone, TV and internet services.
Sentrics connects with Yardi EHR through the FHIR API to populate and update resident data in their platform with resident profile and census updates in Yardi EHR. Yardi EHR clients access automated census management in the Sentrics application.
Teton
Teton helps facilities better navigate the long-term care of their residents with a suite of AI-driven solutions for fall monitoring and alerts, sleep monitoring, event documentation and event analysis.
By connecting to Yardi EHR through the FHIR API, Teton ingests clinical data to create better predictive models and maintain accurate resident population data.
Clients benefit from an automatically synced administrative and clinical interface. Teton automatically updates resident census information based on changes made in Yardi EHR. Clinical data entered in Yardi EHR is sent to resident profiles in Teton, so staff always see consistent resident information across both systems.
Winter 2025-2026 EHR partners
Carefeed
Carefeed is a communication and engagement platform for senior living providers, specializing in automated messaging, digital admission packets and streamlined family communication.
To provide clients with a connected interface, Carefeed and Yardi partnered to deliver real time updates to Carefeed based on data entered in Yardi EHR.
Carefeed absorbs clinical data entered in the EHR in real time and staff can view it there. No one needs to re-enter data in Carefeed, freeing up time for other important tasks.
DiningRD
DiningRD provides dietary consulting, menu solutions and foodservice software for senior living communities, with a focus on nutrition, safety and culinary excellence.
To share data and improve care coordination, DiningRD partnered with Yardi to use the EHR’s FHIR API and ADT interface.
Automated census management eliminates the need to manage resident profiles in two locations. Allergy and dietary information that staff enter in Yardi EHR flows seamlessly to DiningRD for clinical continuity.
SafelyYou
SafelyYou is an AI-enabled fall detection and prevention solution designed specifically for memory care and senior living environments. Its technology combines real-time video review with clinical expertise to reduce fall risk and improve resident safety.
Yardi EHR and SafelyYou clients can use a tailored interface that supports fall-management workflows, aligned with value-based care goals.
With SafelyYou, Yardi EHR clients get increased resident safety with a real-time alert system that prompts post-event workflows following a resident fall. This is prompted by data gathered by SafelyYou resident safety tools.
InTouchLink
InTouchLink is a resident engagement and communication platform that enhances the living experience in senior communities through digital signage, activity management, family communication and educational programming.
To display information from Yardi EHR, InTouchLink uses a Yardi EHR API to bring in census and selected clinical data. This improves visibility for staff and families.
